What was the BTB way to handle reach in AD&D when it concerned monsters (rather then weapons), or did this concept exist back then. I don't particularly care for the way this concept is worked out in 3E, but can't help but wonder if it doesn't already exist in 1E some place.

There were no fixed rules for reach in 1E. In charges weapon length determines first strike using the chart in the PH (note that there's no provision for "extra reach" for larger than man-sized creatures), it's mentioned as an aside once or twice that characters in the second rank with spears or pole-arms might be able to engage in melee, and that's about it. Common sense applies (and the abstract minute-long combat round renders a lot of it moot anyway -- if the ogre wins initiative that means he probably was able to use his reach advantage to gain first strike; if he loses initiative that presumably means the party was able to negate his reach advantage...).
